The Second Affiliated Hospital of Hainan Medical University began its renal transplantation work in May 1982, making it one of the hospitals in China to carry out this technology at an early stage. In January 2008, the hospital took the lead in China to complete an inter-family cross-renal transplantation surgery. This surgery triggered extensive discussions across the country on "cross-renal transplantation" and eventually led to the decision of the Fourth Meeting of the Clinical Application Committee of Human Organ Transplantation Technology of the Ministry of Health: "cross-renal transplantation" between two patient families is legal. Undoubtedly, this has left a significant mark on China's organ transplantation cause.
The Department of Organ Transplantation was independently established on July 31, 2018. On the opening day, it completed 1 liver transplantation and 2 renal transplantation surgeries with high quality through rapid and effective organ transportation, superb surgical skills and skilled team cooperation. Since its establishment, the department has always been guided by discipline construction and pursued development through innovation. The transplant team led by Professor Wang Yi, the academic leader, has carried forward the spirit of forging ahead and made efforts to develop a series of advanced transplant technologies with domestic leading levels. As the first hospital in Hainan with organ transplantation qualification and the only specialized organ transplantation department, the Second Affiliated Hospital of Hainan Medical University is committed to providing more medical treatment opportunities for patients in the province who need transplantation, continuously improving the level of transplant diagnosis and treatment, and taking a firm step towards enabling Hainan patients to receive organ transplantation without leaving the province.
In 2021, the Department of Renal Transplantation became an independent department. At present, the routine surgical projects carried out by the department include renal transplantation (including living-related renal transplantation) and combined pancreas-kidney transplantation. It strives to build itself into a training base for transplant medical talents in the "Belt and Road" countries.
Currently, the department has 35 beds (including 9 ICU beds), 9 transplant doctors and a transplant nursing team of 16 people. It is equipped with 2 ventilators, 1 hemofiltration machine, 1 bronchoscope, 1 color Doppler ultrasound machine. The transplant laboratory is equipped with HLA tissue typing, CDC, PRA, DSA monitoring equipment and FK506 and CSA drug concentration detection equipment.
Renal transplantation, high PRA renal transplantation, living-related renal transplantation, ABO-incompatible renal transplantation, re-transplantation and multiple renal transplantation, combined pancreas-kidney transplantation, combined liver-kidney transplantation, pediatric renal transplantation, management of post-renal transplantation complications, post-renal transplantation infection, and perioperative rehabilitation.
Led by Professor Wang Yi, the team has made outstanding achievements in the research of ABO-incompatible renal transplantation and xenotransplantation, enjoying high prestige at home and abroad. The transplant team has developed a series of advanced transplant technologies with domestic leading levels, and has achieved many "firsts" in organ transplantation in Hainan Province. Its ability in clinical practice, teaching and scientific research leads the province. Meanwhile, under the leadership of Professor Zeng Fanjun, Professor Jiang Hongtao and Professor Wang Jianli, the department carries out living-related renal transplantation and combined pancreas-kidney transplantation.

In the past 5 years, the department has undertaken 20 national/provincial-level projects, including 2 national key R&D program projects and 3 National Natural Science Foundation projects. It has published more than 30 SCI papers and won 2 provincial medical science and technology awards. It has participated in domestic multi-center clinical studies and taken the lead in writing a number of guidelines such as Chinese Clinical Diagnosis and Treatment Guidelines for ABO-Incompatible Living-Related Renal Transplantation (2017 Edition), Chinese Clinical Diagnosis and Treatment Guidelines for ABO-Incompatible Living-Related Renal Transplantation (2023 Edition) and Chinese Clinical Diagnosis and Treatment Guidelines for Invasive Aspergillosis in Renal Transplant Recipients. The Institute of Organ Transplantation plays a crucial role in the organ transplant team. As a large-scale comprehensive institution specialized in basic and clinical research of organ transplantation, it provides technical and scientific research support for organ transplantation.
The department undertakes the teaching tasks for undergraduates and postgraduates of the university as well as the standardized training for resident doctors. It regularly carries out English teaching rounds, weekly professional lectures and scientific research group meetings to improve the scientific research and professional capabilities of teachers and students.
It regularly holds national renal transplantation academic conferences as well as national and provincial continuing education programs. Every year, it regularly holds the Boao Transplant Forum with Hainan transplant characteristics.
Youth meets a prosperous era, and it is the right time to strive. The Department of Renal Transplantation always implements the development slogan of "major diseases can be treated within Hainan Province". It adheres to the guidance of the frontier of organ transplantation discipline development, and takes building a transplant rehabilitation system with high survival rate and high-quality life as its goal. It will establish a regional renal transplantation data center to optimize the matching of donors and recipients; carry out research on cutting-edge technologies such as xenotransplantation; strive to enable all doctors in the department to obtain doctoral degrees, promote the nursing team to gradually improve their academic qualifications, and add 1 doctoral supervisor and 1 master supervisor to enhance the overall academic level of medical staff. The Second Affiliated Hospital of Hainan Medical University is currently the only hospital in Hainan Province with qualification for all major organ transplantations. The Department of Organ Transplantation was established on July 31, 2018. The Department of Liver Transplantation became an independent department on April 23, 2021. It is a provincial key discipline and the Hainan Provincial Liver Transplantation Quality Control Center, which is among the first batch of institutions in Hainan Province to obtain liver transplantation qualification. It integrates medical treatment, scientific research and teaching. The department has professional hospital beds and is equipped with first-class laminar flow clean operating rooms, intensive care units and post-operative rehabilitation centers. It has completed more than 300 liver transplantation surgeries. In recent years, the perioperative survival rate of liver transplantation has reached 100%, ranking among the leading levels in China.
As an excellent talent team in Hainan Province, the Department of Organ Transplantation has gradually grown under the leadership of President Xiao Jinzhu, Secretary Wang Yi, President Xu Jian and Director Zeng Fanjun. At present, the department is led by Director Li Wei, a well-known domestic liver transplantation expert. It has gathered many senior hepatobiliary surgery experts, anesthesiologists, transplant nursing teams and multi-disciplinary cooperation experts. Adhering to the concept of "precision, cooperation and innovation", the team is committed to providing comprehensive and personalized diagnosis and treatment plans for patients with end-stage liver diseases.
The department has achieved many "from zero to one" breakthroughs in Hainan Province, including the first ABO-incompatible liver transplantation, the first adult split liver transplantation, the first living-related pediatric liver transplantation, the first re-liver transplantation, the first combined liver-kidney transplantation and the first liver transplantation with complex vascular variations and other international cutting-edge technologies.
It has successfully treated various patients with complex and difficult liver diseases, such as liver failure with hepatic encephalopathy, genetic metabolic diseases, recurrent liver cancer after resection, patients with re-liver transplantation and patients with liver cancer after immunotherapy.
For patients with liver cancer after immunotherapy, a systematic diagnosis and treatment plan has been formed and has been promoted in many domestic institutions.
A full-process management system of "pre-transplant evaluation - intra-operative precise operation - post-operative immune monitoring - lifelong follow-up" has been established, which has greatly reduced the incidence of complications.

The department undertakes the teaching tasks of standardized training for resident doctors and undergraduate teaching. It has completed the teaching of more than 100 standardized training doctors and trained 6 postgraduates. In 2024, it won the first prize of in-hospital teaching rounds.

Established in June 2021, the Department of Thoracic Surgery Ward 2 (Lung Transplantation Department) of the Second Affiliated Hospital of Hainan Medical University is the chairman unit of the Hainan Provincial Lung Transplantation Quality Control Center, a standardized training base for cardiothoracic surgery and the only department in Hainan Province with lung transplantation qualification. Led by Professor Sun Wei, a well-known national thoracic surgery expert, the department currently has 31 medical staff and 51 open beds. Adhering to the concept of "standardization, professionalism and leadership", the department focuses on standardized, precise and minimally invasive surgical diagnosis and treatment of thoracic diseases. It has set up 4 subspecialties, including Lung, Esophagogastric Junction, Lung Transplantation, and Mediastinum & Chest Wall (covering thoracic trauma), and built a comprehensive ward for thoracic diseases featuring a standardized diagnosis and treatment system for thoracic diseases and lung transplantation technology. Among the medical team, 75% have a master's degree or above, including 1 chief physician, 2 associate chief physicians and 1 master's supervisor; the nursing team includes 1 associate chief nurse and 7 senior nurses. The minimally invasive rate of lung cancer surgery exceeds 95%. Single-port video-assisted thoracic surgery (VATS) covers high-difficulty procedures such as lobectomy, sleeve lobectomy, vascular plasty, mediastinal tumor resection and total thymectomy.
It took the lead in carrying out tubeless thoracic surgery (lobectomy, segmentectomy, mediastinal tumor resection) in Hainan Province.
It conducts precise pulmonary segmentectomy (without pre-operative localization), thoracoscopic and laparoscopic radical esophagectomy, transhiatal esophagectomy, and "tubeless and diet-unrestricted" enhanced recovery after surgery (ERAS) mode.
It performs high-difficulty surgeries such as double sleeve lobectomy, tracheal tumor resection, autologous pericardial plasty for vena cava replacement, intrapericardial pneumonectomy and segmental sleeve resection. Its surgical treatment of chronic empyema and mediastinal abscess leads the province, with grade IV surgeries accounting for more than 90%.
It has completed the first robot-assisted (RATA) radical lung cancer surgery, precise pulmonary segmentectomy and radical esophagectomy (Mckeown) in Hainan Province.
It obtained lung transplantation qualification in 2018.
It has completed a total of 34 cases of single-lung, double-lung, heart-lung combined and ECMO-bridged emergency lung transplants, with patients aged 15 to 72 years old.
In 2021, it successfully performed the first independent lung transplant in Hainan Province.
